White Card or Medical Device User Incident Report Card is a means of collecting information regarding serious incidents that have occurred in Greece from the use of medical devices (Government Gazette 2197B/2-10-2009, Government Gazette 2198B/2-10-2009, Government Gazette 1060B/10-8-2001).
Users of medical devices (healthcare professionals) as well as patients - consumers can report all serious incidents that come to their attention by completing the special incident report card for users of medical devices in Greek (WHITE CARD).
If you are a healthcare professional or a patient-consumer, please use the following hyperlinks to stay informed. To report serious incidents from the use of a medical device, use the button WHITE CARD. Press and print the White Card and then send it completed to MANUFACTURER and to the EOF.
Print, fill out the White Card and send it to:
